12/24/2023 EnchantaVaughn Approved QMHPA arrived to the home of Ms. Brown. Upon arrival to the home, Ms. Brown appeared to be in a positive mood evident by her facial expressions such as a smile on the face while also engaging in general conversation with QMHPA about the session between her and QMHPA. She continued to be joyful indicating to QMHPA that she very content. Upon arrival, she did not present any symptoms of S/I. QMHPA began the session by inquiring about her current thoughts, feelings and emotions. After processing with Ms. Brown about her current thoughts and feelings, QMHPA educated Ms. Brown on productive ways to reduce and begin to eliminate stress through developing healthy relationship with others. QMHPA encouraged Ms. Brown to verbalize her thoughts, feelings and emotions rather than isolating herself from others. QMHPA reinforced to Ms. Brown the importance of learning to communicate respectfully and interact without becoming verbally abusive and socialize with other in a positive manner without becoming aggressive. QMHPA taught Ms. Brown some skills she could explore to help reduce symptoms associated with depression such as exercise, deep breathing and relaxing. Ms. Brown appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. During the session, Ms. Brown was able to identify triggers, length of time she was depressed and the severity of her depression, anger and anxiety. Ms. Brown informed QMHPA that she did not take her medication last night because she just didn’t want to. QMHPA reinforced the importance of a daily medication regime. Ms. Brown was receptive to sticking to a medication regime that will support her mental health once QMHPA discussed with her that it was important for her to be in compliance with her medication. Ms. Brown admits that she is easily frustrated. Ms. Brown also admitted that she has not developed the skills necessary to resolve conflict, listen attentively or respond appropriately. Ms. Brown was able to identify that her anger and frustration contributes to her day-to-day struggles with others.
